Copy of Birth Certificates for New Players Only.  Birth Certificates will be attached to signup forms; however, if top of application indicates that a birth certificate is needed, then you will need to collect from parent.

Completes coaches application forms including insurance information.

Checks for referee funds will not be distributed until all this information is obtained.
